Home Health - MEDICAID - LOUISIANA

This policy covers Medicaid home health services for Humana Healthy Horizons® in Louisiana, including intermittent/part‑time skilled nursing (including EPSDT extended skilled nursing for medically fragile members under age 21), home health aide services, rehabilitation therapies (PT/OT/ST), wheelchair seating evaluations, and medically necessary DME/supplies when specified in the plan of care. Services must be ordered and the POC signed by an authorized healthcare provider, prior authorized by the MCO, delivered by a licensed HHA in the member’s residence on an intermittent basis, meet medical necessity (and minimum intensity for extended skilled nursing), and are excluded in certain institutional settings (e.g., hospitals, generally ICF‑IID) or without required PA.
