Policy Summary
Hospital-based care coordination provided as an In Lieu of Service (ILOS) for pregnant members with substance use disorder enrolled in Louisiana Medicaid (Humana Healthy Horizons). Coverage is limited to adult members (≥18) who are pregnant or up to 12 months postpartum and is subject to specified scope (intake/assessment/care plan, care coordination, outreach), documentation and time requirements, non‑duplication of MCO case management, and potential clinical claims review.
