Humana Medicaid covers certain image-guided injections for chronic pain (epidural steroid injections, sacroiliac joint injections, ganglion impar blocks, and trigger point injections) when specific conservative-treatment prerequisites, diagnostic-response thresholds, imaging guidance, and interval/frequency limits are met. Numerous exclusions and limitations apply (including disallowed procedures such as several peripheral nerve blocks, genicular artery embolization, use of ultrasound for needle placement, and routine use of MAC or sedation by a separate provider for adults), and documentation of conservative care, pain scales, diagnostic response, and timing is required for coverage.
