Coverage is limited to chronic pain indications with required real‑time imaging guidance, session rules (all procedures on a single calendar day), success criteria for diagnostic blocks (typically ≥80% pain relief before confirmatory/ablative procedures), limits on levels injected, and plan‑specific restrictions on anesthesia/sedation and use of ultrasound or MAC.
