This policy covers Magnetic Resonance Angiography (MRA) of the head and neck (e.g., CPT 70544–70548) for diagnostic imaging of cerebral and cervical blood vessels and flow, including stroke evaluation, assessment of internal carotid flow, and high‑risk intracranial aneurysm screening (eg, autosomal dominant polycystic kidney disease or ≥2 family members with aneurysm/subarachnoid hemorrhage). Coverage is limited to the indications and medical‑necessity criteria in the applicable MAC Local Coverage Determination(s) or CMS NCD (jurisdictions without LCD guidance default to LCD L33633), and services that do not meet those criteria are not considered reasonable and necessary.
