Medical Supplies, Equipment and Appliances Durable Medical Equipment - MEDICAID - FLORIDA

This policy covers durable medical equipment (DME)/home medical equipment (HME) and related medical supplies, equipment and appliances (MSEA) for long‑term or everyday therapeutic/home use, including specified HCPCS items (e.g., E0565 compressor, E0575 nebulizer, E0678 compression garment, T5001 positioning seat). It applies to patients who need equipment to provide therapeutic benefit or to enable activities of daily living due to illness or injury, when prescribed by a qualified health care practitioner and not furnished by a hospital or skilled nursing facility. Coverage is subject to medical necessity, plan determination of rental versus purchase, code‑specific rules and exclusions (e.g., certain HCPCS not covered, CPM/traction attachments, home modifications), and customized DME often requires medical director review.

"When the Plan elects to purchase DME, purchase of the item is treated as a covered expense up to the purchase price."
