Humana Medicaid (FL) covers wheelchairs and related medically necessary accessories when a licensed provider's order is present and the member meets general mobility criteria (e.g., neurologic/muscular disorder limiting ambulation, inability to complete ADLs, or being bed/chair confined). Specific coverage rules and additional specialty evaluation and home assessment documentation are required for power, complex rehabilitative, pediatric modifications, power seat elevation, and certain accessories; many accessories, convenience items, home modifications, standing systems, and certain high-end or recreational items are explicitly excluded. Replacement and repair have warranty, cost-comparison, and reasonable useful lifetime (generally 5 years) limitations.
