This policy covers outpatient multiplex pathogen identification panels (NAAT/genetic testing of microbial DNA/RNA) for infectious disease diagnosis under Humana Medicaid (Kentucky), including NAATs for bacterial vaginitis in symptomatic women and respiratory multiplex panels containing six or more pathogen targets. Coverage is limited to plan members and requires specific clinical indications and documentation (e.g., symptomatic respiratory infection that will change management, a negative targeted 2–5 target panel prior to expanded respiratory testing, and high‑risk/immunocompromised status such as active cancer treatment, chronic glucocorticoids, HIV/AIDS, inherited immune deficiency, or transplant); germline/inherited‑condition testing and inpatient use are outside the scope and all coverage is subject to plan criteria and medical director review.
