Negative Pressure Wound Therapy - Medicare Advantage

This policy covers negative pressure wound therapy (NPWT)—including NPWT with instillation and closed‑incision NPWT systems—for treatment of acute and chronic wounds (e.g., surgical wounds, diabetic foot ulcers, pressure and venous leg ulcers) to improve vascularity/oxygenation, reduce edema, promote granulation, or prepare wounds for grafts/flaps. Coverage is governed by applicable Medicare LCDs/MAC jurisdictions and requires Medicare medical necessity (patients must have comorbidities preventing expected healing with standard topical treatments); continuation is limited (generally an additional 30 days) only with documented wound improvement, and NPWT is excluded for conditions such as exposed vital organs, fistulas to organs/cavities, malignant or necrotic wounds with eschar, exposed vessels or nerves, and untreated osteomyelitis.
