Indications include paraplegia (T7–L5 when accompanied by a trained caregiver; T4–T6 limited to rehabilitation institutions), select spinal conditions (C1–2 rotatory subluxation pre‑fusion, severe thoracolumbar deformity), unstable fractures, and certain elbow and lower‑extremity disorders, but coverage requires a physician prescription, clinical documentation of medical necessity and appropriate therapy, is subject to the member’s plan and device‑specific institutional/caregiver restrictions, and typically allows replacement every 1–3 years.
