HUMANA-OUTPATIENT-LACTATION-SUPPORT-PROVIDER-IN-LIEU-OF-SERVICES-ILOS-LA-MEDICAID
Covers outpatient lactation support (individual and group breastfeeding education and assistance, including milk expression for infant nutrition) provided as an In Lieu of Service for pregnant and postnatal Louisiana Medicaid members through 24 months postpartum. Services must be furnished by credentialed lactation providers (e.g., IBCLC, ALC, ANLC, CLC, CBS, CLE, or breastfeeding peer counselors), are limited to a maximum of six sessions per member with group classes capped at eight unique persons, require proof of credentialing on claims, and are subject to Humana Healthy Horizons LA ILOS/billing rules and member eligibility.
