Skin and Tissue Substitutes - MEDICAID - KENTUCKY
HUMANA-SKIN-AND-TISSUE-SUBSTITUTES-KY-MEDICAID
This policy covers bioengineered skin and tissue substitutes for management of wounds, burns and soft-tissue reinforcement—including neuropathic diabetic foot ulcers, venous ulcers, first/second-degree burns, and use in breast reconstruction. Coverage is limited to specified indications (for example diabetic foot ulcers typically must be full- or partial-thickness >4 weeks with no bone/capsule/muscle/tendon exposure), generally requires prior inadequate response to standard wound therapy and product-specific restrictions (e.g., required compression for venous ulcers or association with a covered breast reconstruction) and is conditional on Kentucky Medicaid member eligibility.
