HUMANA-SKIN-AND-TISSUE-SUBSTITUTES-SC-MEDICAID
This Medicaid South Carolina policy covers skin and tissue substitutes (including amniotic membranes, Wharton’s jelly and other biologic allografts/cellular matrices) for management and repair of acute and chronic wounds such as venous and diabetic foot ulcers, partial- and full-thickness wounds, surgical sites, tissue defects/voids, and wounds with exposed bone, muscle, or tendon. Coverage is subject to product-specific limitations and requirements (e.g., Apligraf for venous ulcers requires standard compression), many named products are excluded or restricted (e.g., Integra, Oasis, Dermagraft, certain Grafix and Apligraf variants), and billing/coverage may depend on HCPCS coding and the member’s individual coverage document.
