This policy covers transcatheter peripheral (non‑coronary) vascular stent placement in the chest, abdomen and pelvis to restore or improve blood flow for stenotic or occluded arteries and veins, including conditions such as brachiocephalic/subclavian disease (eg, subclavian steal), celiac artery compression/MALS, chronic mesenteric ischemia, hemodialysis AV access stenosis, hepatic venous outflow obstruction (Budd‑Chiari) and other peripheral vascular lesions. Coverage is governed by applicable CMS NCDs/LCDs/LCAs (with MAC LCDs applied by jurisdiction), generally requires that PTA alone is unlikely to provide a durable result, and may be superseded by member‑specific coverage terms; angiography/IVUS are described as adjuncts to PTA/stent procedures.
