This policy covers ultraviolet phototherapy and photochemotherapy (including NB‑UVB, BB‑UVB, PUVA) and targeted laser procedures for dermatologic conditions such as psoriasis and atopic dermatitis. PUVA is generally reserved as a second‑line therapy after failure of conventional treatments, use is cautioned in patients with fair skin (Fitzpatrick I–II), prior arsenic or ionizing radiation exposure, personal history of melanoma or multiple nonmelanoma skin cancers, recurrent oral herpes, or lupus, and final coverage is subject to applicable CMS NCD/LCD/LCA requirements.
