This policy covers vagus nerve stimulation (VNS) therapy — an implanted pulse generator and lead system that delivers intermittent stimulation to the left vagus nerve — primarily for treatment of seizures in individuals with epilepsy not controlled by antiseizure drugs and as a nonresective surgical option per CMS NCD 160.18. Coverage is limited by applicable Medicare NCDs/LCDs and Medicare Advantage rules, excludes investigational or non‑FDA‑approved devices (eg, SetPoint and integrated bioelectronic systems), and lists other uses (treatment‑resistant depression, bipolar disorder, PTSD, stroke rehabilitation, autoimmune diseases, etc.) as proposed or investigational rather than established covered indications.
