HUMANA-VARICOSE-VEIN-TREATMENTS-MA
Covers treatment of varicose veins of the lower extremities (including sclerotherapy and management of chronic venous insufficiency) as specified by the applicable Medicare Administrative Contractor (MAC) LCD/LCA, generally for symptomatic, clinically significant disease—typically dilated/tortuous veins >3 mm and/or reflux ≥0.5 seconds (including GSV/SSV-related reflux). Coverage is limited to the indications, criteria and billing/coding rules in the applicable MAC LCD/LCA and CMS NCD within the MAC’s geographic jurisdiction; cosmetic, asymptomatic, or subthreshold disease (e.g., reflux <0.5 s or veins ≤3 mm) and other specific limits/frequencies are determined by the current LCD/LCA and related coding guidance.
