This Humana Healthy Horizons (Medicaid Louisiana) policy covers provision and basic repairs of manual and motorized (power) wheelchairs, standing frames, and patient lifts for eligible enrolled members—primarily those who are bed-, chair- or room‑confined, not functionally ambulatory, or have severe upper‑extremity weakness or neurological/muscular conditions preventing manual propulsion and requiring long‑term (≥6 months) motorized mobility. Coverage requires prior authorization and documented medical necessity (prescription, seating evaluation, signed/dated clinician verification that the member has the cognitive, motor and perceptual abilities and can be trained to safely operate a motorized wheelchair); only one wheelchair at a time is authorized, backups are generally denied, and repairs are limited to basic component fixes.
