Covers wound care supplies, surgical dressings/bandages and wound care systems for Humana Healthy Horizons Medicaid members in Louisiana to manage wounds—including stage III/IV chronic nonhealing wounds (pressure, venous stasis, diabetic ulcers), postsurgical wound dehiscence, non‑adhering skin grafts, surgical flaps, wounds requiring infection assessment, and members using pressure‑reducing mattresses/cushions. Prior authorization is required (approvals issued in three‑month intervals), an approved home health agency must be involved, prescriptions must be updated for extensions, supplies must be part of a documented prescribed wound care regimen, and certain items (e.g., portable hyperbaric oxygen chambers placed directly over the wound) are not covered.
