Policy Summary
Medicare covers therapeutic shoes and related inserts when they are Medicare-eligible items and are reasonable and necessary for diagnosis/treatment or to improve functioning of a malformed body member, with therapeutic shoes typically intended for persons with diabetes. Custom-molded shoes (A5501) are covered only for foot deformities not accommodated by depth shoes and require clear documentation; suppliers must obtain and retain a signed written order (WOPD/SWO) prior to delivery and maintain proof of delivery and coding compliance or claims will be denied.
