Transcranial Magnetic Stimulation

Repetitive transcranial magnetic stimulation (rTMS) is considered medically necessary for adults (>=18) with confirmed major depressive disorder (single or recurrent) who have failed or cannot tolerate adequate trials of psychopharmacologic therapy (typically from at least two agent classes), have undergone an adequate trial of evidence-based psychotherapy without improvement documented by standardized rating scales, and meet provider, device, and safety criteria. Contraindications include seizure history (with limited exceptions), active psychosis, certain neurological conditions, and implanted magnetic‑sensitive devices or metal within 30 cm of the coil; maintenance TMS is not covered and d‑TMS for OCD is specifically not considered reasonable and necessary.
