Policy Summary
Medicare covers Mohs micrographic surgery for specified biopsy‑proven skin malignancies (including BCC, SCC, basalosquamous carcinoma and a list of rarer tumors) and for tumors in high‑risk anatomic locations or with high‑risk features. The performing physician must be trained in MMS and document that the diagnosis is appropriate and MMS is the most appropriate treatment; biopsy is generally required and biopsy/pathology performed within 60 days prior disallows separate payment unless the surgeon documents inability to obtain results despite reasonable effort.
