Chemodenervation with botulinum toxins and other chemical neurotoxins is covered for multiple neurologic, muscular, glandular, and sphincter disorders (e.g., blepharospasm, cervical dystonia, limb dystonia/spasticity, strabismus, achalasia, anal fissure, sialorrhea, severe primary hyperhidrosis, detrusor overactivity/OAB, and certain headache disorders). Headache prophylaxis coverage is limited to adults with chronic migraine meeting frequency/duration thresholds (≥15 days/month with ≥4 hours/day) and intractability criteria; onabotulinumtoxinA (BOTOX®) is the only product FDA‑approved for chronic migraine prophylaxis and potency units are not interchangeable between products.
