Policy Summary
CT of the thorax is covered for a wide range of diagnostic and interventional indications involving the lungs, mediastinum, pleura, chest wall, cardiovascular structures (including suspected aortic dissection), and for CT-guided biopsies or drainages when fluoroscopy is inadequate. Coverage requires documentation of medical necessity (preferably preceding chest x-ray or physical exam findings), use of FDA-recognized CT equipment in full market phase, and performance/interpretation by qualified radiology personnel; screening CTs are not covered.
