CCTA/MDCT cardiac CT is covered for diagnostic evaluation of symptomatic patients when CAD is suspected (including chest pain, equivocal stress tests, known CAD with new/recurrent symptoms, post-CABG or post-stent evaluation, suspected congenital anomalies, pre-procedural coronary or pulmonary vein mapping, and select ED presentations with nondiagnostic ECG/enzyme results). CCTA is not covered for screening or when it will not provide new management-changing information, when invasive angiography will obviously be required, or for isolated quantitative calcium scoring; performance and interpretation require specific credentialing, physician supervision, and appropriate technologist training.
