Visual Fields Testing

Formal visual field testing (typically automated perimetry) is medically necessary for a range of ocular, optic nerve, retinal, orbital, and intracranial conditions — including documented glaucoma, suspected glaucoma with supporting signs, unexplained visual loss, medication-related toxicity risk, recent intracranial events or masses, and after significant eye injury. Gross confrontation visual field testing is part of the general ophthalmologic exam and is not separately reportable; testing frequency is individualized based on IOP changes, retinal damage, and optic disc findings, with required documentation of the clinical indication and relevant exam or imaging findings.
