Transthoracic echocardiography (TTE) is covered when performed for accepted clinical indications to evaluate ventricular function, valvular, pericardial, and great vessel disease, congenital heart disease, transplant monitoring, cardiotoxicity surveillance, suspected embolic sources, and for stress testing when results would affect management. Coverage requires adequate documentation, permanent records, appropriate credentialing of technical and interpreting personnel, and adherence to specified frequency limits (e.g., transplant, chemotherapy, prosthetic valve follow-up); routine screening and many annual re-evaluations in asymptomatic, stable patients are not covered.
"TTE is indicated to evaluate ventricular function, cardiomyopathies, and heart failure when history, physical exam, or routine labs suggest or cannot exclude cardiac disease."
