Transesophageal echocardiography (TEE) is covered when TTE is technically inadequate or when TEE is expected to provide additional diagnostic information that will significantly affect patient management (examples: endocarditis, prosthetic valve dysfunction, atrial thrombus, aortic dissection, intra-procedural guidance, critically ill patients). Coverage requires performance by qualified personnel, documentation of permanent records and written interpretation, and is conditional—routine screening, monitoring-only intraoperative use, or studies performed by unqualified providers are not covered.
