Policy Summary
Osteopathic manipulative treatment (OMT) is covered when medically necessary, performed by a qualified physician (D.O. or M.D. trained in OMT), and the medical record documents somatic dysfunction (one or more TART findings) with expectation of clinical or functional improvement. OMT can address somatic components of visceral disease, but is not covered without documented somatic dysfunction; E/M services are separate but should not be billed for routine, previously planned follow-up OMT visits unless a new condition or substantial change is documented.
