Policy Summary
Ophthalmic biometry (A-scan ultrasound, B-scan when dense cataract, or optical coherence biometry) is covered for IOL power calculation when planning cataract surgery and when a decision for extraction has been made by the patient and surgeon. The technical component is generally performed and reimbursed bilaterally and is valid for 12 months (repeat scans within 12 months require documentation of significant vision change or an exception for a different unaffiliated surgeon); the professional component is unilateral and billed for the operative eye only.
