Injection therapy into tendon sheaths, ligaments, ganglion cysts, and carpal or tarsal tunnels is covered to relieve substantial pain or significant functional disability when conservative measures have failed, are contraindicated, or not appropriate; carpal tunnel injections are limited to mild–moderate symptoms after conservative therapy has failed. Services must be preceded by a thorough diagnostic evaluation and performed by appropriately trained and credentialed providers; billing counts all injections at a single anatomic location on the same date as one service and general anesthesia is contraindicated for diagnostic blocks.
