Medicare covers corneal pachymetry when it is integral to medical management and performed by an ophthalmologist or optometrist for specific clinical indications including assessing endothelial trauma from surgery, preoperative evaluation in Fuchs dystrophy, post-ocular trauma, and measurement of corneal thickness in suspected glaucoma after documented increased intraocular pressure prior to initiating treatment. Pachymetry for refractive surgery preparation is not covered, and glaucoma-related pachymetry is generally limited to one lifetime test unless additional testing is justified by interval corneal trauma or surgery.
