Duplex scanning is covered for evaluation of arterial inflow and venous outflow abnormalities across abdominal, pelvic, scrotal, renal, portal, aortic/iliac, and grafted vasculature when clinical signs, symptoms, trauma, or post-procedural surveillance indicate likely utility. Routine use is limited to situations where duplex provides necessary anatomic information (eg, post-trauma vascular injury, suspected aneurysm, renovascular hypertension, portal hypertension, graft surveillance), and it is not the preferred primary test when other modalities (eg, chest x‑ray, TEE, aortography) are recommended, particularly for aortic dissection. Coverage is subject to Medicare reasonable-and-necessary determinations and contractor LCDs.
