Automatic external defibrillators (wearable K0606 and nonwearable E0617) are covered for beneficiaries at high risk of sudden cardiac death when specific clinical criteria are met (documented VF or sustained VT, inherited high‑risk arrhythmia syndromes, prior MI or cardiomyopathy with defined LVEF thresholds, EP-study–documented inducible VT/VF with required timing, or explantation of a prior device). Coverage excludes transient or reversible causes, acute MI period constraints, and beneficiaries with recent revascularization procedures, cardiogenic shock, irreversible neurologic injury, or other noncardiac illnesses with life expectancy <1 year; required documentation (SWO/WOPD, POD) and adherence to coding guidelines are mandatory for payment.
