Peripheral venous ultrasound is covered for evaluating suspected acute DVT (signs/symptoms), investigation of DVT as the source of confirmed pulmonary embolism, follow-up when clinical change would alter treatment, evaluation of chronic venous insufficiency or postthrombotic syndrome, post-ablation assessment, and preoperative vein mapping for bypass grafting. Routine screening, imaging that will not affect management (including most imaging while adequately anticoagulated), asymptomatic varicose veins, certain plethysmography methods per NCD 20.14, and same-day multiple duplex studies are not covered except in rare, documented circumstances; documentation (Wells score, D-dimer when required, hard-copy output, clinical rationale, provider qualifications) must support all claims.
