Visual field examinations are covered when clinically indicated for a range of ocular, optic nerve, retinal, orbital, neurologic, vascular, congenital, traumatic, medication‑related, or functional vision problems (including documented glaucoma or glaucoma suspects and central vision ≤20/70). Exclusions include routine preoperative or follow-up visual fields for cataract patients without glaucoma or glaucoma suspicion and no pretreatment field required for established retinal detachment; frequency of testing is individualized (especially for glaucoma) and repeat exams for macular degeneration are only indicated for documented changes or postoperative evaluation.
