Policy Summary
Destruction of malignant skin lesions is covered when a pathology report confirms a skin malignancy or when the lesion's clinical description in the medical record is consistent with malignancy. Coverage is subject to the requirement that the service be reasonable and necessary under Social Security Act Section 1862(a)(1)(A) and to any contractor Local Coverage Determinations that define applicable circumstances.
