Policy Summary
Surgical excision with appropriate margins is the preferred and medically necessary treatment for malignant skin lesions (e.g., BCC, SCC, melanoma) when a pathology report verifies malignancy. Coverage requires documented histopathology and is subject to contractor Local Coverage Determinations and the requirement that the service be reasonable and necessary under SSA Section 1862(a)(1)(A).
