Policy Summary
Trigger point injections (local anesthetic and/or steroid) are covered for treatment of trigger points that are refractory to non-invasive therapies or when non-invasive treatments are contraindicated, provided the medical record documents prior treatments and responses. Repeat injections are allowed when there is documented clinical benefit, but generally no more than three injection sessions per year are supported and further injections into the same muscle are not recommended after two to three ineffective attempts.
