Peripheral Nerve Blocks

Peripheral nerve blocks are medically reasonable and necessary for diagnostic and therapeutic management of localized peripheral nerve pain syndromes (e.g., mononeuritis with nondiagnostic studies, entrapment syndromes, occipital and trigeminal neuralgia, CRPS) and for perioperative analgesia including single-shot and continuous techniques. Coverage excludes use for diabetic peripheral neuropathy or neuropathies from systemic diseases (including electrostimulation) as investigational, and imposes frequency limits: no more than three injections per anatomic site in six months and no more than two anatomic sites per session.
