Policy Summary
Routine foot care may be covered for patients with peripheral neuropathy of the feet without vascular impairment when neuropathy is severe enough that non‑professional care would place the patient at risk. Coverage requires documentation of absence of sensation at ≥2 of 5 sites on either foot using the 5.07 Semmes‑Weinstein monofilament, and providers must document heightened risk for patients on anticoagulation; final coverage is subject to contractor reasonable‑and‑necessary determinations per Medicare rules.
