25-hydroxyvitamin D testing is covered for patients with specific conditions (e.g., CKD stage III+, osteoporosis, osteomalacia, osteopenia, hypo-/hypercalcemia, parathyroid disorders, malabsorption, cirrhosis, rickets, low sunlight exposure, obesity, and to monitor vitamin D replacement). Routine screening of Medicare beneficiaries is not covered and assays of vitamin D metabolites other than 25-hydroxyvitamin D are generally not medically necessary. After documented deficiency, testing is limited to monitoring adequacy of replacement, and annual testing may be appropriate based on clinical circumstances.
