Computerized corneal topography is covered when needed to evaluate corneal surface irregularities that affect management — examples include pre-operative assessment for cataract IOL power when irregular astigmatism is documented, monocular diplopia due to corneal irregularity, early keratoconus, post-penetrating keratoplasty, post-surgical/post-traumatic astigmatism (≥3.5 D), corneal dystrophies, graft complications, corneal scarring, pterygium, and corneal ectasia causing visual impairment. The test is not covered for routine screening or follow-up and repeat testing is allowed only if a patient reports a change in vision related to one of the listed indications; providers must document clinical need and notify beneficiaries in writing when coverage is uncertain.
