Ultrasound of the head and neck is covered when used to evaluate palpable masses or other tissue/organ abnormalities, to investigate findings from other imaging (including thyroid scans), for patients with personal or family history of thyroid cancer, to assess suspected regional nodal metastases in proven thyroid carcinoma, for nodule follow-up, and to localize thyroid/parathyroid glands or cervical lymph nodes for interventional procedures. Coverage requires documentation of the clinical indication and compliance with ordering provider qualifications per 42 CFR §410.32; contractors/LCDs may impose additional reasonable-and-necessary criteria or restrictions.
