Policy Summary
RAST, FAST, multiple antigen simultaneous tests, and ELISA for specific IgE are covered when a complete history and physical exam demonstrate clinical necessity for evaluation of IgE-mediated allergy to specified allergens. Tests that are non-specific or measure IgG (including ELISA/Act qualitative antibody testing, LMRA by ELISA/Act, qualitative multi-allergen screens, and IgG/IgG-subclass food antibody tests) are considered not medically necessary and will be denied.
