Medicare covers surveillance (in-person or remote) of implantable or wearable ICDs to monitor device behavior, investigate discrete symptoms (post-shock, syncope, palpitations), assess suspected device malfunction, and to program or disable devices for procedures. Remote monitoring (codes 93295/93296) is billed as a single, inclusive 90-day service (one per 90 days) that includes any in-person interrogations within that period; in-person interrogation codes (93287, 93289, 93292) may be billed each time if not included in a remote 90-day period. Coverage is limited to FDA-approved systems for the specific ICD model, requires documented medical necessity, physician or qualified practitioner prescription, and signed physician interpretation/reports with appropriate supervision.
