Injections of local anesthetics, steroids, sclerosing, or neurolytic agents into ganglion cysts, tendon sheaths/origins/insertions, ligaments, or near nerves of the foot (including Morton's neuroma) are covered to relieve pain or dysfunction, and carpal/tarsal tunnel injections are covered for mild cases. Treatment should be short-term as part of a diagnostic and overall management plan (typically 1–3 injections), injections beyond three to the same site within six months or more than two sites per session require documented justification, and dry needling is non-covered; records may be audited via post-payment review.
