Electrocardiograms (12‑lead ECGs and rhythm strips) are covered to diagnose or treat symptoms, signs, or history of heart disease and systemic conditions affecting the heart, including ischemia/MI, arrhythmias, heart failure, hypertension, pericarditis, and drug cardiotoxicity. Preoperative ECGs are reasonable when there is pre‑existing heart disease, comorbid conditions that may affect the heart, or when the planned surgery requires general or regional anesthesia. The ECG recording must be reviewed by a physician who provides an interpretation and written report and may be billed as technical only, interpretation only, or both.
