External ocular photography is covered when medically necessary to photo-document and follow progression or treatment response of conditions affecting the external eye or anterior segment, particularly when magnified images of lesions are obtained using a special camera. Coverage requires medical-quality images with patient identification and date permanently affixed, an interpretation/report in the medical record, and photography is billable only once per session; images obtained solely for medicolegal or preauthorization purposes, or merely to enhance the medical record, are not separately reimbursable.
