Visual Fields Testing

Medicare covers formal visual field testing (typically automated perimetry) when there is clinical indication that visual fields may be affected, including glaucoma diagnosis or suspicion, optic nerve/retinal/neurologic visual pathway disorders, recent intracranial events or vascular occlusion, significant eye injury, unexplained visual loss, medication toxicity risk (e.g., hydroxychloroquine), congenital or orbital conditions, or functional limitations suggestive of field loss. Gross (confrontation) testing is part of the general ophthalmologic exam and should not be billed separately; documentation requirements include written beneficiary notification when a service may not be covered and adherence to limitation-of-liability/refund rules where applicable.
